HealthEast Diabetes Care

HealthEast Diabetes Care offers education and training to help you safely manage your blood sugars - whether you have type 1, type 2 or gestational diabetes. Since 1993, we have met the national standards for diabetes education.

Our diabetes educators work with patients at HealthEast hospitals and at most HealthEast Clinics locations. Education is available one-on-one or in a group setting. Family members are welcome, too.

Diabetes educators provide training on:

  • Blood sugar (glucose) testing
  • Managing your medications
  • Meal planning and healthy eating
  • Exercise and weight management
  • Motivation and stress management
  • Emotional adjustment to a diabetes diagnosis
  • Preventing or delaying complications

Diabetes services

  • Managing insulin
    Through this outpatient program, you learn the skills necessary to safely manage the use of insulin.

  • Managing type 2 diabetes
    Approximately 90% of all cases of diabetes are people with type 2. This comprehensive program provides you with the skills and knowledge to safely manage type 2 diabetes through diet, exercise, oral medication or insulin.

  • Managing pregnancy and diabetes
    We provide medical care and education to women who have diabetes during pregnancy, whether it's gestational or pre-existing. Through Diabetes Care, you will have the opportunity to consult with a doctor who specializes in diabetes. We teach you the skills to help you have a safe pregnancy and a healthy baby.

  • Pre-diabetes program
    This program will give you information and support to make changes. We'll discuss the importance of healthy blood sugar goals, healthy eating and being more active.

Working together

In partnership with your doctor, our staff of professionals - certified diabetes educators, pharmacists and exercise specialists - offer education and training to help you and your family manage diabetes.

We see patients at most HealthEast Clinics locations. To make an appointment, please call your clinic directly.

Insurance

Most of our programs require either a clinician's recommendation or referral. Health insurance companies typically cover a portion of the cost for the services provided.

If you don't have insurance, you may qualify to have a portion of the services covered through a scholarship program established by the Lions 5M-6 Clubs and HealthEast Foundation. To learn more, call 651-232-5757.
